Job Summary:
The Patient Transitions Facilitator will support the philosophy that each patient receives the right level of care in the right place at the right time by the right provider. This role will promote optimal patient discharge outcomes by facilitating the effective management, utilization of resources and length of stay and will address issues impeding the patient’s flow from admission to discharge. The Patient Transitions Facilitator will co-ordinate the multidisciplinary team’s plan of care to ensure needs are met in a timely manner and consistent messaging to the patient and family regarding discharge plans occurs. This role will coach and empower patients to become advocates for their own care using pathways and will also endorse inter-professional collaboration related to length of stay.
Requirements:
- Active registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse is required.
- Completing or willingness or complete Foundations of Geriatrics Course and Core Geriatric Knowledge Course
- Minimum of five (5) years of recent acute care experience is required.
- Experience with discharge planning and patient flow.
- Experience in emergency, critical care nursing is considered an asset.
- Demonstrated ability to work collaboratively in a multidisciplinary team environment including internal and external partners is required.
- Demonstrated strong leadership ability is required.
- Demonstrated facilitation, critical thinking and problem-solving skills is required.
- Demonstrated exceptional organizational skills are required.
- Demonstrated ability to be self-directed and thrive in a dynamic environment is required.
- Active and proven engagement in continuous quality improvement is required.
- Experience with Meditech is required.
- Knowledge of Microsoft Office is required.
- Demonstrated knowledge of the various bed types within Blanche River Health, individual admission criteria unique to each unit, i.e., Acute Surgery/Medicine, Alternate Level of Care/Long Stay, Mental Health and Palliative Care etc. is preferred.
- Bilingualism in French and English is preferred.
